Washington DC: War Department, 1864. Large printed Civil War order form for soldiers uniforms with manuscript signatures of 56 members of the Company (3 signed with an X), and titled: "No. 52. WE the undersigned Non-commissioned Officers, Artificers, Musicians, and Privates, of Company "I" 12th Conn Arty do hereby acknowledge to have received of Capt. John H. Burton the several articles of Clothing set opposite our respective names". There is a John H. Burton, age 29, listed as a member of the Consolidated List of persons in the 2nd Congressional district of New Haven, CT in July/August 1863, who has been promoted to 1st Lieutenant.

The names of the soldiers are recorded in the furthest left column, followed by uniform coats, jackets and other items, with the signatures of the soldiers and the signature of the witness, N. F. Stoughton, at the far right. The items ordered total: 6 forage caps; 5 flannel sack coats; 21 Privates' trousers; 25 flannel shirts; 10 pairs drawers; 6 pr cavalry boots; 7 pr infantry bootees; and 32 pr of stockings. One printed panel on verso, with printed statement exhibiting the clothing allowance to each soldier during his enlistment, with ink notation, "Receipt Roll No. 1, retained copy".

Capt. Burton is recorded in Hamden CT on the 1880 Federal Census, born in Scotland with his wife Candace C. and 6 children. He received a pension in 1892 (application No 1116935) and his wife a widow pension in 1896 (No 633205). Sergeant James P. Elliott has signed for his clothing allowance. According to the Hale Collection of Cemetery Inscriptions he was born 18 Dec 1835, d. 5 Apr. 1909. His wife was Ellen S. Sgt. Hubert G. Scott (27 Dec 1838 - 3 Feb 1906) & his wife Cornelia are listed in the Hale collection as well. Others include Corporal Luke Tiffany (5 Feb 1842 - 29 Jan 1916) and George D. Oliver, age 36 & 46 in the 1870/80 census for Providence, RI, a railroad brakeman then engineer. His wife Louisa is also listed.

The regiment served in the defenses of Washington DC in 1862; at the Battle of Chancellorsville and Gettysburg; rejoined defenses of Washington in 1864; Bermuda Hundred; Petersburg and Richmond; Fort Dutton; Fort Brady, James River; Fort Fisher NC; Petersburg Va; returned to defenses of Washington DC in 1865.
